A Bit About Us

The Futures Planning and Support pilot launched on the Mid North Coast, NSW in April 2020. This pilot is service led by Burrun Dalai Aboriginal Corporation in partnership with Uniting. The pilot aims to support ALL young people aged between 17 -25  years  transitioning from Out Of Home Care (OOHC) and/or those who have left statutory care.

Services

Intensive Case Management

  • Young People with highly complex circumstances receive intensive support coordination to reach their own goals. This service will target identified priority cohorts, our service will target all Culturally diversity, of young people, young parents, and those who have lived in residential care.

Futures Coach

  • The Futures Coach is an adviser of mentor who keeps in touch with the young person, helps them access services, provides advise to support them reach their goals and achieve their identified outcomes, Medium and higher risk young people and priority populations also receive additional FPS coordination. They can have access to a Futures Coach, who can request reviews or additions to Futures Plans. For Aboriginal young people the Futures Coach may be provided by an Aboriginal Futures Coach and/or be a respected community member identified by the young person or community.

Universal Support

  • All care leavers are entitled to a range of universal services (like, income support) and specific entitlements outside of this pilot. They should have access to their leaving care plan and information about mainstream services. They should be supported with referrals to other services which links directly to goals, needs and aspirations set out in the Leaving Care Plan.
